Vacation Care Bookings

How do I book?

  1. Programs and booking forms are available approximately four weeks prior to each school holiday period.
  2. Download the program (PDF 2.3 MB) and select the days and activities you wish your children to attend.
  3. Download the enrolment form (PDF 275 KB) and complete all details clearly.
  4. If your child has anaphylaxis or nut allergies you will need to submit a current Anaphylaxis Action Plan for your child with your enrolment (if you have not already supplied a current copy).

Note:

  • Enrolments will not be processed unless all details are provided.
  • Please ensure you provide your Customer Reference Number (CRN) and date of birth on enrolment forms.

You can lodge your enrolment in the following ways:

  • email all forms to cityofryde@ryde.nsw.gov.au
  • fax all forms to 9952 8070
  • post to: City of Ryde, Locked Bag 2069, North Ryde NSW 1670
  • pay in person at Customer Service Centre, Civic Centre, 1 Devlin Street, Ryde.

You can also drop off your completed forms with a credit card authorisation or cheque to any of the City of Ryde Library branches.  Payments cannot be processed at the library. All forms are forwarded to Customer Service for processing. 
Please Note: Enrolments received after the specified date will incur a late fee.

Allergies, Asthma and Other Conditions

You need to inform staff if your child:

  • suffers from mild to moderate allergies
  • suffers from severe allergies (anaphylaxis)
  • suffers from specific food allergies eg. nuts or eggs
  • suffers from asthma
  • suffers from any other medical condition.

You need to provide a health care plan which would inform staff on how to manage any allergic reactions or asthma attacks.
